In order to be eligible, the patient must:

one

Male or female, 18 years or older with a current urgent medical need that requires specialty care

Eligibility

Living in Greater New Haven; Branford, Northford, Woodbridge, East Haven, New Haven, Orange, Guilford, North Haven, Wallingford, Hamden, North Branford or West Haven

Eligibility

Eligible patient must have no medical insurance including Medicaid, Medicare, or government benefits, male or female that is 18 years or older.

Household income less than or equal to 250% of the Federal Poverty Level (FPL)

Project Access New Haven referrals must be made by a healthcare provider

Requirement

Referrals may be submitted by fax using the PA-NH Referral Form or other referral template
OR electronically through Epic (the Yale New Haven Hospital electronic medical record)

Referrals MUST include relevant past medical history (e.g., imaging, labs, treatment)

  • Volunteers can determine their level of commitment by selecting the number of PA-NH patients they are able to see
  • Patient care is coordinated and administrative “hassles” are eliminated
  • Patients have a high show-rate for appointments (96%)
  • Ancillary services (e.g., diagnostics, inpatient procedures) are available and coordinated by PA-NH
  • Ability to help those most in need in our community

PA-NH provides donated medical services to patients of limited insurance status. Patient Navigators assist patients with appointments and navigation of the health care system. Barriers to access are addressed so that physicians can focus on delivering the highest quality care.
