REACH is a national program administered by the Centers for Disease Control and Prevention (CDC) to reduce racial and ethnic health disparities. Through REACH, local programs offer culturally appropriate services to help individuals address a wide range of health issues. Project Access-New Haven (PA-NH) has partnered with the Community Alliance for Research and Engagement (CARE), Southern Connecticut State University (SCSU), and many other community partners to implement REACH in New Haven.
A PA-NH REACH community health worker (CHW) plays a key role in this program by linking participants with clinical resources for primary and preventative healthcare services and community-based resources for health-related social needs.
