Breast Health Navigation Program

BREAST HEALTH NAVIGATION PROGRAM

Eligibility and Referral Information

All patients receive patient navigation services, including: assistance scheduling and coordinating timely appointments for breast cancer diagnostic testing and treatment, reminders for medical appointments, assistance addressing barriers to care, and support navigating the healthcare system.

Uninsured patients receive free breast health services.

All PA-NH patient navigators are bilingual in English and Spanish.

HOW DOES IT WORK?

Step

one

PATIENTS MUST BE REFERRED

Patients must be referred to PA-NH from a healthcare provider. Referred patients are assessed for eligibility and those who qualify meet with a PA-NH Patient Navigator for an intake assessment. Referral forms can be found on our website at www.pa-nh.org.

PATIENT NAVIGATOR

Patient Navigators help patients obtain needed services and navigate a complex, confusing, and often intimidating health care system. They also work with patients to remove barriers and ensure access to needed care – for example, by assisting with the coordination of translation or transportation services.

FREE CARE IS PROVIDED

Medical care is provided free of charge to patients through the Project Access-New Haven network, which includes Yale New Haven Hospital (YNHH), Yale Medicine (YM), Northeast Medical Group (NEMG), independent medical practices throughout New Haven, and other local healthcare providers and community partners.

18 years of age or older

A resident of the Greater New Haven Area (Branford, East Haven, Guilford, Hamden, Miilford, New Haven, North Branford, North Haven, Northford, Orange, Wallingford, West Haven, Woodbridge)

Patient Must Be

Uninsured (Note: Insured patients may be accepted on a case by case basis. Eligibility will determined based on the above criteria and the patient’s need for navigation to address one or more of the following barriers: language, low health literacy/difficulty understanding medical information, and lack of social/emotional support. Insured patients will receive navigation services at no cost, but medical care will be billed to their insurer. Please call PA-NH for more information)

Referred from a healthcare provider for breast cancer diagnostic testing or treatment.

Below 250% of the Federal Poverty Level

Patient must maintain regular communication with PA-NH patient navigators

Patient must keep all appointments arranged by PA-NH

Inform PA-NH immediately about changes to insurance status, income, or address

Patient Must

Undergo financial screening every 6 months to re-confirm eligibility

Patient Must

Complete initial and 1 year follow-up interviews

Patient Must

Consider giving back through a testimonial or volunteering

WHO IS ELIGIBLE?

In order to be eligible, individuals must:

Abnormal finding on a breast cancer screening (e.g., mammogram, physical exam) or a breast cancer diagnosis.

HOW TO REFER A PATIENT?

OR electronically through Epic (the Yale New Haven Hospital electronic medical record)

PA-NH provides donated medical services to patients of limited insurance status. Patient Navigators assist patients with appointments and navigation of the health care system. Barriers to access are addressed so that physicians can focus on delivering the highest quality care.
