Project Access New Haven is growing!

Project Access-New Haven (PA-NH) opened its doors in the fall of 2010 with one referral site (Fair Haven Community Health Center, FHCHC), one full-time employee (our Executive Director), one part-time Patient Navigator, and a small office in donated space provided by the Hospital of Saint Raphael (HSR).

Although we started small, our mission has always been big: improve access to healthcare for uninsured residents of the Greater New Haven area by providing timely access to quality specialty and ancillary care and by coordinating the delivery of services across the continuum of care. Thanks to the generosity of our physician volunteers, Yale-New Haven Hospital (YNHH), HSR, and our many funders and donors, we experienced considerable growth and made substantial strides towards achieving our goals in 2011.

In our first year of operation, we served over 200 individuals and coordinated the delivery of more than $1,000,000 in donated medical care. Given minimal operating costs during this time, we were able to leverage $4 in delivered services for every dollar spent on the program. Referrals for specialty care covered more than 20 medical and surgical specialties and patients were provided with a broad range of ancillary services, including anesthesia, imaging and laboratory services, physical therapy, and chemotherapy.

In addition to FHCHC, we are now accepting referrals from the Primary Care Centers at YNHH and HSR and the YNHH Emergency Department. We have also partnered with YNHH to launch a Demonstration Project that will evaluate the impact of our program on patient outcomes and the local health care system, as well as assess the applicability of our model for larger and broader populations – for example, the Medicaid population.

Our staff now includes two Patient Navigators, an RN Patient Navigator, a Director of Research and Evaluation, and Program Coordinator, in addition to the Executive Director. Needless to say, we have outgrown our office in HSR, but are thankful to the Hospital for providing a space from which to launch our program. Beginning January 1, 2012, the PA-NH office will be located at 150 Sargent Drive in a space generously donated by YNHH.

While we have accomplished much, there is still a great deal of work to be done. Plans are currently underway for expansion to the Cornell-Scott Hill Health Center and HSR Emergency Department and we are looking forward to continued growth and development in 2012 and beyond.
