Communities Joined in Action Conference, Washington D.C.

September 30, 2015-PA-NH was a sponsor of the Communities Joined in Action annual conference in Washington D.C. Staff and a founding Board member attended the 2 day conference that brought together community leaders from multiple sectors and walks of life from across the nation to learn with and from one another.

Project Access-New Haven Receives $200,000 Grant for Patient Navigation

June 8, 2015-The Connecticut Affiliate of Susan G. Komen and Walgreens presented a check to Project Access-New Haven (PA-NH) for the Breast Health Navigation Program. The grant to PA-NH will provide patient navigation to underserved women in Greater New Haven who have abnormal results from breast cancer screenings. Services will be designed to promote timely access to and utilization of appropriate diagnostic testing and treatment, link patients with health and social resources and educate and support patients and their families as they move through the breast cancer continuum of care.
