Patient Survey Interviewer Position Description
The Patient Survey Interviewer will be responsible for conducting telephone surveys with individuals who have received services through Project Access-New Haven (PA-NH). Specific responsibilities will include reviewing patient charts to obtain information about the services provided to the patient by PA-NH, following a standard protocol of patient outreach (via phone and mail) to request survey participation, conducting a structured survey over the phone, entering patient responses into an electronic survey system, and recording outreach attempts and tracking survey participation in an electronic patient database.
This is an exciting opportunity to work with a local nonprofit organization that is providing needed healthcare services to underserved individuals living in the Greater New Haven area, as well as contribute to a vital program evaluation/health services research initiative. The Patient Survey Interviewer will be directly involved in the evaluation of PA-NH and our unique service delivery model. He or she will work under the Director of Research and Evaluation and receive training on patient outreach and data collection protocols.
Individual must be bilingual in English and Spanish, have excellent communication skills, and be able to work independently with regular support and supervision from PA-NH staff. Position requires flexible weekday hours as well as some evenings and/or weekends. Basic computer skills and ability to document progress in an electronic format also needed. Background or interest in healthcare, public health, social services, health services research, program evaluation, or survey methods preferred, but not required.
